Slip disc leading to sciatica is one of the most common problems affecting humans. Dr Himanshu Tyagi is senior consultant Orthopaedic and spine surgeon practice in Delhi. In this video he talks about slip disc (prolapsed intervertebral disc) & sciatica and their treatment. Slip disc (PIVD) is most common in lumbar spine and cervical spine and can lead to symptoms like numbness, tingling ,muscle weakness and sometimes even paralysis. It can even lead to rare problems like cauda equina syndrome where nerves controlling motion and urine are affected. Sometimes surgery is the only option for treatment when there is a muscle weakness. However 95% of slip disc (PIVD) cases can be treated without surgery by just conservative treatment in the form of medicines and exercises and sometimes spinal nerve root injection.

Conservative treatment for lumbar or cervical slip disc (PIVD) includes medicines like analgesics, anti-inflammatory, muscle relaxant and nerve pain medicines for initial few days followed by exercises in the form of spinal Extension and core muscle strengthening exercises.

Selective nerve root block (SNRB) is a procedure which is done in operation theatre under local anaesthesia where we selectively target the nerve pressed by slipped disc (prolapsed intervertebral disc ) causing sciatica or radicular pain. Selective nerve root block is a safe and effective procedure.

Surgical treatment for slip disc is best through endoscopic discectomy which is a 40 minutes procedure and patient needs to stay in the hospital for two hours only. It is a totally safe and effective procedure as it is done under local anaesthesia and there is a minimal risk for infection or blood loss. During this procedure the patient is totally awake and talks to the surgeon. In endoscopic discectomy we specifically target the disc fragment which is pressing the nerve and causing sciatica pain. We do not cut muscles during endoscopic discectomy and the scar is smaller then the fingernail.
